Home Health Services

At Advanced Home Care, our patients have access to nearly 500 nurses, therapists, social workers and home health aides, all of whom are dedicated to providing the highest-quality, most compassionate home care available.  We work with each of our patients and their physicians to develop an individualized plan of care that emphasizes education and encourages independence and self-reliance.

All of our home health personnel are licensed professionals who operate according to their individual profession’s state board practice acts.  We perform background checks, including a criminal background check, on each of our employees and also check to make sure their license is valid and up-to-date.

All of our employees complete an orientation prior to working with patients and also engage in ongoing education and competency training.  In addition, Advanced Home Care is licensed by each state in which it operates. Advanced Home Care’s Home Health Solutions include:

Medical Social Therapy (click for details)

The medical social workers at Advanced Home Care help patients and their families solve any financial or social problems that are interfering with the process of recovering from an illness, surgery or life-changing event.All of our medical social workers are trained in social work or a related field and have received any additional licensure as required by the state in which they operate. Some areas with which our social workers can assist include the following:

  • Preparing advanced directives, such as a living will or healthcare power of attorney
  • Contacting community service programs, such as Meals on Wheels or medication assistance programs
  • Applying for federal or state assistance programs, such as Medicaid
  • Overcoming unsafe, neglectful or potentially violent home environments

Occupational Therapy (click for details)

At Advanced Home Care, our goal is to help patients regain the highest level of functionality and independence possible. Occupational therapy is geared towards helping patients better perform the activities of daily living, such as:

  • Using the bathroom
  • Opening doors
  • Eating
  • Bathing
  • Dressing oneself
  • Light housework
  • Preparing meals
  • Taking medication

Our program begins with an individualized evaluation to determine each patient’s specific needs. Then, in conjunction with the patient’s physician, our occupational therapists create a personalized plan of care designed to restore the patient to a maximum level of independence in the activities of daily living. A safety and functionality assessment is also performed at the patient’s home to make sure it best meets his or her rehabilitation goals. Finally, our occupational therapists can also teach energy conservation to patients with chronic diseases, such as Congestive Heart Failure, helping to improve their quality of life.

Physical Therapy (click for details)

Helping patients regain strength and functional ability are key goals of Advanced Home Care’s team of highly skilled physical therapists.  Our in-home rehabilitation therapies are individually developed to address each patient’s unique condition and take into account his or her goals and special needs.

We work with patients who have suffered a stroke or undergone an orthopedic surgery, in addition to patients who have chronic diseases such as Congestive Heart Failure (CHF) or Chronic Obstructive Pulmonary Disease (COPD).  Our programs are designed to improve patients’ physical abilities, increase their strength capabilities and make the activities of daily living easier to manage.

Some of the services we provide patients include the following:

  • A complete physical assessment of the patient and the patient’s environment
  • Communication with the patient’s physician regarding goals and progress
  • The establishment of an individualized plan of treatment with input from the patient and the patient’s caregivers
  • The establishment of a home maintenance program
  • Individualized training and instruction to patients and their caregivers
  • Safety training
  • A complete range of therapeutic exercises that are individualized to meet the particular needs of specific patients

Skilled Nursing (click for details)

Advanced Home Care has more than 270 highly skilled nurses (RN) and licensed practical nurses (LPN) available to provide in-home healthcare to patients as needed.  Our skilled nurses evaluate patients’ health and provide an individualized treatment plan to address the patient’s specific needs.  Most importantly, they are able to answer questions and teach patients and caregivers the skills they need to regain maximum health potential and independence.

Some of the services our skilled nurses provide include the following:

  • Disease management
  • Wound, ostomy and continence management
  • Venipunctures
  • Care and maintenance of urinary catheters (see #2)
  • Administration of intravenous, intramuscular and subcutaneous medications
  • Care of central lines
  • Enteral and parenteral nutrition
  • Patient and family education
  • 24/7/365 on-call availability for emergency situations

Wound Management (click for details)

Advanced Home Care’s wound management program ensures the best outcomes for patients with surgical incisions, infections or wounds that are slow to heal, including pressure ulcers.

We have three board-certified wound ostomy continence nurses who educate patients about proper wound maintenance, including signs and symptoms of infection, and identify the best treatment options, such as the following:

  • Compression therapy for lower extremity wounds
  • Debridement (removal of dead tissue)
  • Negative pressure wound therapy to accelerate healing
  • Non-invasive vascular assessments
  • Topical wound dressings

In addition, we use digital photography to document wounds and assess the treatment process.

Our wound ostomy and continence nurses also identify other conditions that can cause or contribute to slow-healing wounds, such as:

  • Arthritis
  • Blood flow problems
  • Diabetes
  • Infection
  • Kidney disease
  • Non-compliance with prescribed treatments
  • Poor nutrition and diet
  • Smoking
  • Swelling
  • Weight

Our staff is also specially trained to match ostomy patients with the pouching system that best fits their lifestyle, saving them both time and money.
