Skilled Nursing & Assisted Living Facilities

Advanced  Home Care offers Home Medical Equipment (HME), Respiratory, Enteral and Infusion supplies and services to skilled nursing and assisted living facilities. In doing so, patients can stay within the Advanced Home Care community to meet their needs during transitions between the hospital, the facility and the home.

Skilled Nursing Facilities

Advanced Home Care partners with skilled nursing facilities in the following ways:

  • To coordinate care for patients transitioning from a facility to the home setting
  • To provide facilities with medical supplies and equipment for their patients

Assisted Living Facilities

Advanced Home Care partners with assisted living facilities by caring for their residents through:

  • Providing skilled services – such as home health, nutrition, respiratory and infusion therapy
  • Delivering necessary medical supplies and equipment
