COPD Program

What is COPD? Chronic Obstructive Pulmonary Disease (COPD) is an illness that interferes with how the lungs work and makes breathing difficult.

Symptoms of COPD:
  • Difficulty breathing
  • Shortness of Breath
  • Wheezing
  • Excess Mucus
  • Coughing
  • Weakness and exhaustion
  • Weight Loss

Market Overview: There are 14-16 million patients in the United States diagnosed with COPD and an estimated 14-15 million who are undiagnosed. The number of patients using long term oxygen therapy has grown at a rate of 10% per year since 1995 and that trend is expected to continue, yielding 2.5 million home oxygen patients by 2005 and approximately 200,000 new home oxygen patients per year.

Mission Statement: The mission of this program is to improve patient clinical and quality of life outcomes through the implementation and management of a care plan. The program focuses on patient self-management. It is intended to serve as a pathway to assist physicians and their patients improve management of COPD by focusing on patient self-management.

The AHC COPD Program is designed to benefit the patient and physician. The patient receives additional visits to educate about their disease, how and when to take medications, breathing tips and exercises, and nutrition. When program is completed, the physician will receive a patient outcome summary report. This report demonstrates the patient progress between the first and the last contact.
